Acknowledgement

Please type your name below acknowledging that you understand that if payroll deduction is selected, CSISD will deduct the amount specified from your salary each pay period and remit to the CSISD Education Foundation. This is a tax-deductible charitable contribution and you will need to keep an account of my balance paid for each year for claiming a deduction on your individual tax return. The deduction amount will appear beside the letters ED FND on the check stub. This authorization shall become effective upon receipt and shall remain in effect until the CSISD Business Office receives a written notice of cancellation or change at least 20 days prior to the next scheduled payday.
